GDPR and Your Data

Please read through carefully. This will allow you to understand why we are collecting your personal information, what we are going to do with it and where it is going to be stored. All personal information collected by our administration team is used to arrange your appointment. 

• They will enter this information into our OH management system to set up the appointment, send you confirmation of the appointment and to issue any outcomes for the appointment directly to you
• This information will not be used to send you any information that is not directly linked to your appointment with Granite OH.

All personal information collected by our clinical team is used to determine the outcome of the appointment

• Whether it be a fitness to work medical to determine your fitness to carry out your job or a management referral to ensure that your work is not affecting your health, we only collect information that is relevant to that particular appointment.

We do not release any details of the appointment outcome without your consent, this includes certificates and reports to your employer. 

• At no time will confidential medical details be released to your company without informed consent being requested from you by the clinical team
• The release of information is done so electronically using our OH management system or via encrypted email

All of your personal and medical information is stored within our secure OH management system

• Access to each section of the OH management system is restricted
• Your employer will only be able to access released certificates and reports
• The only paper copies of information held by Granite OH are what is required by the HSE. For e.g. hard copies of medical certificates following a medical for Ionising Radiation, Lead or Asbestos.
